Identification of neoplastic cells in peripheral blood by flow cytometry in patients with B-cell non-Hodgkin's lymphoma

Introdução: Os linfomas não Hodgkin (LNH) são as neoplasias hematológicas mais comuns no mundo. Novas técnicas tem sido estudadas para permitir a identificação mais ráoida desta doença como a imunofenotipagem por citometria de fluxo. Em relação ao sangue periférico, até o momento não há consenso sobre o desempenho da citometria de fluxo neste contexto e o seu real valor para o diagnóstico e estadiamento dos LNH é desconhecido. Métodos: Analisamos se é possível identificar a presença de células neoplásicas circulantes no sangue periférico de pacientes com LNH por meio da imunofenotipagem por citometria de fluxo, bem como comparamos os achados com o estadiamento inicial por biópsia da medula óssea. Amostras de sangue periférico de nove pacientes com LNH foram avaliadas usando um painel de anticorpos monoclonais para detectar distúrbios linfoproliferativos crônicos de células B. Resultados: Neste estudo observamos discordância entre os resultados da biópsia de medula óssea e a citometria de fluxo em sangue periférico em três das nove amostras estudadas. Os casos discordantes mostraram positividade para neoplasia na imunofenotipagem do sangue periférico e negatividade na imuno-histoquímica da biópsia de medula óssea. Nestes casos, as células neoplásicas circulantes corresponderam a menos de 5% da celularidade da amostra, inferindo maior sensibilidade. Conclusão: O uso de sangue periférico no context de estadiamento e monitoramento de LNH permanence pobremente investigado mas os achados deste estudo permitem definer a importância da imunofenotipagem na identificação precoce e sensível de células neoplásicas em pacientes com LNH.
