DETERMINANTES SOCIAIS DA SAÚDE, TERRITÓRIO E GOVERNANÇA: LACUNAS ENTRE POLÍTICAS PÚBLICAS E IMPLEMENTAÇÃO EM CONTEXTOS DE INIQUIDADE

Os determinantes sociais da saúde (DSS) constituem um campo consolidado de investigação científica e de formulação de políticas públicas, mas a persistência de iniquidades em saúde em distintos contextos nacionais evidencia um hiato estrutural entre o enunciado normativo das políticas e sua efetiva implementação. Este artigo apresenta uma revisão integrativa da literatura com o objetivo de analisar criticamente as interseções entre determinantes sociais da saúde, territorialidade e governança em saúde, problematizando os mecanismos pelos quais condições estruturais de classe, raça, deficiência, orientação sexual, gênero e localização geográfica produzem e reproduzem desigualdades em saúde. A busca foi realizada nas bases PubMed, Scopus e Web of Science, com recorte temporal entre 2021 e 2025, e resultou em um corpus analítico de 25 artigos, complementados por três referências metodológicas. Os resultados indicam que as lacunas entre política e implementação são recorrentes e estruturalmente condicionadas, manifestando-se em contextos tão diversos quanto Gana, Austrália, Irã, Indonésia e Estados Unidos. A discussão aponta para a insuficiência de abordagens técnico-gerenciais que não problematizem as dimensões políticas e históricas das iniquidades, defendendo uma perspectiva de equidade em saúde como imperativo de justiça social. As considerações finais apontam para a necessidade de reformulação dos marcos legais, de integração intersetorial das políticas e de participação comunitária como condições indispensáveis à superação das desigualdades persistentes.
