Abstract

Introduction: Pain, agitation and elevated blood pressure are common and difficult-to-control manifestations during recovery from anaesthesia. Dexmedetomidine, an alpha-2 agonist with sedative and analgesic properties, may show promise in mitigating these events. This study assessed its impact on the recovery from anaesthesia in patients undergoing laparoscopic cholecystectomy. 

Methods: A randomised, single-blind clinical trial involving 50 patients undergoing elective laparoscopic cholecystectomy under balanced general anaesthesia. Participants were allocated to two groups: the dexmedetomidine group (DG; n = 25), which received an infusion of 0.5 mcg·kg⁻¹·h⁻¹, and the control group (CG; n = 25). Age, sex, BMI and ASA score were analysed. Sedation and agitation were assessed using the Riker scale on admission to the operating room (T0) and after extubation (T1). Following the intervention, mean arterial pressure (MAP), heart rate (HR), pain (VAS), nausea and vomiting, the need for rescue propofol, time to extubation and length of stay in the post-anaesthesia care unit (PACU) were assessed. A p-value < 0.05 was considered statistically significant.

Results: The groups were similar in terms of clinical and anthropometric characteristics, except for a higher proportion of ASA II patients in the CG (88% vs. 56%; p = 0.012). Following extubation, the CG had a higher number of agitated patients (68% vs 36%), with a statistically significant difference in the median Riker scale scores (5.00 [2.0] vs 4.00 [1.0]; p = 0.016). Only in the CG were there patients with a Riker score of 7 (4%), whilst 12% of patients in the CG had a Riker score of 3. Pain was more prevalent in the CG, both upon awakening and during the post-anaesthesia recovery period, although this was not statistically significant. A significant reduction in mean arterial pressure (MAP) was observed only in the CG (p = 0.008). HR remained stable in the GD group (p = 0.841), whilst there was an increase in the GC group (p = 0.196). There was no statistical significance regarding time to awakening or length of stay in the post-anaesthesia care unit.

Conclusion: Dexmedetomidine reduced agitation, pain and blood pressure levels upon emergence from anaesthesia, without significantly increasing the time to wake-up or the length of stay in the post-anaesthesia care unit.
