Introduction: It is well known that managing chronic diseases represents one of the greatest challenges for healthcare systems worldwide, particularly due to their high prevalence and the ongoing need for long-term care. In this context, in recent years, telemedicine has emerged as a promising tool to improve the accessibility and efficiency of healthcare services. This study aims to explore the impact of telemedicine on the continuity of care for patients with chronic diseases, evaluating its applicability, especially in the Brazilian context. Methodology: This is an integrative review on the proposed topic, based on full scientific articles in Portuguese or English, sourced from databases such as PubMed, SciELO, Virtual Health Library, and others, within the time frame from 2014 to 2020, using Health Sciences Descriptors (DeCS) to select the studies that best fit the research. Results and Discussion: Telemedicine has the potential to transform the continuity of care for patients with chronic diseases by facilitating continuous monitoring and communication between patients and healthcare professionals, regardless of geographical location. This positive impact can be observed in various dimensions of care, from improving treatment adherence to reducing serious complications associated with these conditions. Furthermore, the implementation of telemedicine in the care of chronic patients can also lead to a significant reduction in exorbitant healthcare costs. However, although telemedicine undoubtedly offers numerous benefits for the continuity of care, its implementation also faces challenges, particularly in a country with high rates of economic and social inequality like Brazil. Conclusion: Telemedicine and telehealth, in general, are innovations with immense potential to revolutionize the Brazilian healthcare system, but they still require adaptations to become means of equity rather than tools that further accentuate the inequalities within the population.
