Spinal cord involvement in sjogren's syndrome

Abstract

Sjögren's syndrome (SS) is a chronic autoimmune disease that causes inflammation of the exocrine glands, with a slow and benign course. Extraglandular involvement can involve any organ in the body. The study aims to identify the profile of patients with SS and spinal cord involvement, identify the most affected spinal cord site on imaging exams, the prognosis after completion of therapy and compare similarities and differences with a case report. A systematic review of the literature on spinal cord involvement in SS was carried out. The PUBMED, SciELO and LILACS databases were used to select the scientific articles included in the review, using the descriptors “Sjögren's syndrome”, “Sjögren”, “neurological” and “spinal cord”, and interconnected by the Boolean operator “AND”. After applying the filters and analyzing the titles and abstracts of the articles, 64 articles were read in full and the eligibility criteria applied. At the end of this process, eight articles were selected for review. In addition, a case report of a patient admitted to a university hospital was prepared, and a comparison was made with the results obtained from the systematic literature review. Spinal cord involvement in SS is more prevalent in female patients, with a mean age of 54.45 years. The cervical region of the spinal cord is most affected, visualized by magnetic resonance imaging as enhancement on T2. ​​The main therapy consisted of glucocorticoids, leading to stability of the condition or reduction of symptoms. The case report presented differs only in relation to age and glandular symptoms. SS can manifest with spinal cord involvement, but there is a lack of information on this subject. Therefore, it is concluded that new studies and descriptions of clinical cases are necessary in order to expand knowledge on the subject and to establish a standardized treatment approach.
