Abstract

Objective: To evaluate whether menopausal hormone therapy (MHT) can serve as a preventive strategy against metabolic syndrome (MetS) in postmenopausal women, based on the most recent scientific evidence. Methods: This integrative literature review was conducted between February and April 2025, utilizing the PubMed, SciELO, and LILACS databases. The descriptors "menopausal hormone therapy," "estradiol," "estrogen," and "metabolic syndrome" were used, combined with Boolean operators. Inclusion criteria included full-text articles published within the last five years. Exclusion criteria comprised articles unrelated to the topic, duplicates, or non-scientific works. A total of nine articles were analyzed in full. Discussion: The studies indicate that the use of estradiol, either alone or in combination with progestogens such as drospirenone, provides significant metabolic benefits, including reduced insulin resistance, decreased visceral adiposity, and improved lipid profiles. These positive effects are more evident when MHT is initiated before the age of 60 and/or within 10 years of menopause, corroborating the "window of opportunity" hypothesis. Conclusion: When prescribed early and on an individualized basis, MHT may play a protective role against MetS. However, its indication must take into account cardiovascular risk and individual contraindications.
