DESPRESCRIÇÃO DE MEDICAMENTOS EM PACIENTES HOSPITALIZADOS: ANÁLISE DE ELEGIBILIDADE E IMPACTO ECONÔMICO POTENCIAL

Deprescribing is a key strategy for the rational use of medicines, particularly in hospitalized patients exposed to polypharmacy and increased risk of adverse drug events. This study aimed to assess deprescribing eligibility and the potential economic impact of benzodiazepines, antipsychotics, and proton pump inhibitors (PPIs) in hospitalized patients at a philanthropic hospital in Campo Grande, Brazil. This was a cross-sectional, observational, and quantitative study conducted between March and November 2025, including 52 adult patients using at least one medication from the target classes. A total of 83 medications were analyzed, considering that patients could use multiple therapeutic classes simultaneously. Deprescribing eligibility was assessed using internationally validated clinical algorithms (deprescribing.org), and the economic impact was estimated based on the total cost of medication units administered during hospitalization. A high prevalence of polypharmacy was observed, with prescriptions ranging from 2 to 20 medications per day. PPIs accounted for the largest proportion of medications and showed the highest deprescribing eligibility, mainly due to lack of clear clinical indication. Antipsychotics and benzodiazepines presented lower eligibility, generally associated with conditions requiring continued therapy. The total estimated cost was R$ 1,902.94, of which R$ 1,346.19 (70.74%) corresponded to medications eligible for deprescribing. Intravenous omeprazole 40 mg accounted for the largest share of potential cost savings. These findings indicate that deprescribing has substantial potential to optimize pharmacotherapy and reduce hospital costs, particularly related to PPI use.
