Oroantral communication: clinical aspects, diagnosis and therapeutic approaches in dental practice

Oroantral communication (OAC) is a relatively frequent clinical complication in dental practice, particularly associated with the extraction of posterior maxillary teeth due to their close anatomical relationship with the maxillary sinus. When not properly diagnosed and treated, it may progress to complications such as chronic maxillary sinusitis, negatively impacting the patient’s quality of life. In this context, the present study aimed to analyze, based on recent scientific literature, the main clinical aspects, diagnostic methods, and therapeutic approaches used in the management of oroantral communication, highlighting the importance of early diagnosis for treatment success. This study consists of an integrative literature review conducted through searches in the National Library of Medicine (PubMed), Scientific Electronic Library Online (SciELO), and Virtual Health Library (BVS) databases, including publications from the last five years. Clinical studies, case reports, and reviews addressing diagnostic and therapeutic interventions related to OAC were included. The findings indicate that the choice of treatment technique is directly associated with the size of the communication, the duration of the lesion, and the presence of concomitant sinus infection. Among the therapeutic approaches analyzed, grafts associated with Platelet-Rich Fibrin (PRF) showed favorable outcomes, with high success rates and shorter healing time. It is concluded that the management of oroantral communication should be individualized and evidence-based, emphasizing the importance of accurate diagnosis and proper treatment planning in dental practice.
