THE INFLUENCE OF STRESS ON THE PATHOPHYSIOLOGY AND MAINTENANCE OF CHRONIC PAIN

A dor crônica é um fenômeno multidimensional que transcende a lesão tecidual, sendo influenciada por mecanismos psicofisiológicos complexos. O objetivo deste estudo foi analisar, por meio de uma revisão integrativa, a influência do estresse na fisiopatologia e na manutenção da cronicidade álgica. A busca foi realizada na base de dados PubMed, abrangendo o período de 2022 a 2026, utilizando descritores controlados (MeSH) e filtros para o topo da pirâmide de evidência (Ensaios Clínicos Aleatorizados, Revisões Sistemáticas e Metanálises). Após o processo de triagem baseado nas diretrizes PRISMA, foram selecionados 17 artigos. Os resultados evidenciam que o estresse crônico atua como um mediador crítico na sensibilização central e na neuroinflamação, onde variáveis como o estresse pós-traumático, a raiva e a percepção de injustiça potencializam a interferência da dor na funcionalidade. Observou-se que intervenções voltadas à modulação do estresse, como o exercício aeróbico e o mindfulness, apresentam eficácia na modulação da dor e na melhora do prognóstico clínico. Conclui-se que o manejo do estresse é indispensável para a prática fisioterapêutica fundamentada no modelo biopsicossocial, sendo um fator determinante para interromper o ciclo de persistência da dor e promover a recuperação funcional.
