Use of computed tomography in resolving a complex case - Lower premolar with apical bifurcation and extensive periapical lesion

Abstract

A 43-year-old patient reported pain on chewing and vertical percussion in the right lower second premolar. Radiographically, anatomical variation was observed, with two roots exhibiting apical bifurcation. Given this variation, a cone-beam computed tomography scan was requested to plan endodontic therapy. Extensive periapical bone radiolucency of approximately 17 millimeters in length and apical bifurcation were observed. Cervical preparation was performed with an Orifice shaper #25/09. Root canal preparation was performed with a Solla Collors S1 file associated with irrigation with sodium hypochlorite. Obturation of the root canal system was performed with a calibrated gutta-percha cone associated with AH-Plus cement and thermoplasticization with a #50 gutta-percha condenser, according to Tagger's hybrid technique.
