Abstract

Pneumonia remains one of the leading causes of morbidity and mortality from infectious diseases worldwide, representing a major public health concern due to its high rates of hospitalization, mortality, and healthcare costs. This study aimed to analyze the epidemiological profile of pneumonia-related hospitalizations in Brazil between 2020 and 2025. A descriptive ecological study was conducted using secondary data extracted from the Hospital Information System of the Brazilian Unified Health System (SIH/SUS), available through the Department of Informatics of the Unified Health System (DATASUS) via the TABNET platform. The variables analyzed included geographic region, sex, age group, race/ethnicity, number of hospitalizations, deaths, and mortality rate. During the study period, 3,460,743 hospitalizations and 378,888 deaths due to pneumonia were recorded nationwide. A reduction in hospitalizations was observed in 2021, followed by a progressive increase through 2025. The Southeast region accounted for the highest number of hospitalizations (1,322,333) and deaths (177,897), as well as the highest mortality rate (13.45). Hospitalizations and deaths were more frequent among males, although mortality rates were similar between sexes. Individuals aged 60 years and older accounted for the highest number of hospitalizations (1,599,539), concentrated most deaths (316,051), and presented the highest mortality rate (18.82). Regarding race/ethnicity, individuals identified as mixed-race ("Pardo") showed the highest absolute numbers of hospitalizations and deaths, whereas Black individuals exhibited the highest mortality rate. These findings demonstrate that pneumonia remains a major cause of hospitalization and death in Brazil, particularly among older adults and residents of the Southeast region, highlighting the influence of demographic and social factors on disease distribution. The results emphasize the need to strengthen preventive strategies, expand vaccination coverage, promote early diagnosis, and reduce inequalities in access to healthcare services in order to lessen the burden of pneumonia on the Brazilian Unified Health System.
