Complete repair of extensive periapical bone radiolucency of the right lower first molar after 1 year of clinical and radiographic follow-up

Abstract

A 45-year-old female patient was referred to an endodontic specialist for endodontic treatment of tooth 46. During the anamnesis, an inactive fistula and no symptoms to cold thermal stimulation were observed in both teeth. Radiographically, extensive periapical radiolucency was observed. Coronal opening was made with a 1014 round bur and canal exploration with C-Pilot files 10 and 15. The working length was determined with an apex locator, followed by irrigation with 2.5% sodium hypochlorite. Root canal preparation was performed with a Reciproc Blue R25 file in the mesial canals and R50 in the distal canal. The root canal system was obturated using the single cone technique associated with AH-Plus Jet cement. Clinical and radiographic follow-up was performed three, six and eight months after canal obturation. It is concluded that endodontic treatment determined clinical and radiographic success, since extensive periapical bone radiolucency neoformation occurred.
